ASSISTED LIVING
Sachem Assisted Living Program serves those residents who are medically eligible for nursing home placement but is able to serve those needs in a less medically intensive, lower cost setting.
The Assisted Living Program (ALP) is an adult home that is certified by the New York State Department of Health to provide Medicaid–funded home care services.
The Assisted Living Program is an alternative to nursing home care that enables nursing home eligible people to receive home health care services in the adult home, delaying or preventing the need for admission to a nursing home.
Home care services are provided by an on-site licensed home care agency operated by Sachem ALP and a community certified home health agency under contract. Imagine the comfort of remaining in a familiar environment with established relationships even when health needs increase and assets may be depleted.
